Went to an oral surgeon and paid $250 out of pocket right then and there. Just now, I see that there is a claim on my insurance that I owe $250 for the CT scan. Will this be cleared? I’m not very sure about how insurance works.

The way my insurance works is the company will send me a little note that says I COULD owe an amount. Usually I wait for the physicians bill to confirm. The bill is unlikely to the insurance company, it’s their best guess as to what wasn’t covered or what the provider will require payment on. The insurance companies do not always know what you paid immediately. Edit: I say all this loosely on personal experience. If you are concerned you owe, call the doctor and your insurance for clarification. Not worth a collections credit hit over $250.
